Privacy Policy

The Personal Information Protection Act (“PIPA”) regulates the collection, use and disclosure of personal information by private sector organizations in British Columbia. “Personal information” means information about an identifiable individual, including an employee. This funeral home recognizes the importance of privacy and the sensitivity of personal information received by us in the provision of funeral service. We recognize we have an obligation to maintain our clients’ confidentiality and also recognize our obligations concerning the personal information of our employees.

 

 

COLLECTION, USE AND DISCLOSURE OF PERSONAL INFORMATION

 

PIPA provides that we can collect, use and disclose personal information in accordance with the provisions of PIPA. We recognize our obligation to protect personal information. We have therefore made arrangements to make sure that personal information we have about our clients and employees will not be used improperly.

 

 

REQUEST FOR ACCESS TO PERSONAL INFORMATION

 

PIPA permits individuals, including clients and employees, to submit written requests to us to provide them with:

 

(a) their personal information under our control; or

 

(b) information about how their personal information which we have has been and is being used by us; and

 

(c) names of any persons to whom we have disclosed personal information of the client and/or employee.

 

We will respond to access requests within the time permitted by PIPA and will make reasonable efforts to assist applicants to make sure our response is as accurate and complete as is reasonably possible.

 

LIMITATION ON ACCESS TO PERSONAL INFORMATION

 

The Act provides that we must not disclose personal information if:

 

(a) disclosure would reveal personal information about another individual;

 

(b) disclosure would reveal the identity of an individual who has provided personal information about another individual in circumstances where the providing person does not consent to disclosure;

 

(c) disclosure could cause significant harm to the safety or physical or mental health of the individual who makes the request;

 

(d) disclosure could threaten the safety or physical or mental health of some individual other than the individual who made the request.

 

There are other more technical provisions in PIPA setting out some circumstances where we are not required to disclose personal information. Where such a situation arises, we will advise the person making the request of the reason why we are not required to disclose personal information.

 

 

CORRECTING PERSONAL INFORMATION

 

PIPA permits individuals, including clients and employees, to submit written requests to us to correct errors or omissions in their personal information that we have in our records. Upon receipt of such a request, we will make the appropriate corrections, and if reasonable to do so, send correction notifications to other organizations to whom we disclosed incorrect information, but we do reserve the right not to correct the personal information, but rather to annotate it to note that a correction was requested but not made.

 

 

CLIENT PERSONAL INFORMATION

 

Our policy with respect to client personal information is essentially the same as our policy with respect to employee personal information, with the exception that we as a policy will obtain from the client a consent form, in the form attached.

 

 

SECURITY OF PERSONAL INFORMATION

 

We recognize an obligation to protect personal information during the course of operating our funeral home. We have therefore made arrangements to secure against the unauthorized access, collection, use, disclosure, copying, modification or disposal of personal information.
